Short synopsis: a US missile station on Okinawa got sent the attack codes during the Cuban crisis. It was only the skepticism of the commander (who wondered why some of his targets were *not* in the USSR) that saved the day. He queried the veracity of the codes up the chain of command and the error was thus detected.
Without this fine gentleman, none of us might be here to have this discussion....
