Thanks GUMP, I had been intending to use that event for just that reason, but couldn't lay hands on the report at the time I did the diagram. Fits the pattern totally, especially the phrase in the report summary about "A SERIES (my emphasis) of causal factors" leading to loss of control of both engines.
Many other qualifying events are also in the IFALPA Polaris awardees list
