I have virtualy the entire set of the'Aviation Safety Digest'. It's facinating stuff and great reading. What amazes me is the way the the same sorts of accidents happen again and again... The only real problem with the 'Digest is that the authors and editors embraced the "pilot error" model of accident causation. This model is outmoded as a means of determining the root (ie:systemic) causes of accidents as it it provides an inadequate basis on which to run an accident prevention program. Great reading tho. I love the gory pictures from the mid '70s. Esp. the mustering accidents.
