I don't know of any made solution, but without trying to be funny would it not be better, (probably easier) and cheaper to do it yourself?
In the two company's I have worked for (operating different types) they have both had their own selection of questions which normally focus on points to learn from issues that have happened on the line or common misunderstandings.
