I have taken them in the US and they are administered by the FAA. You can ask the CFI to contact the FSDO for advice, but the best they would do would be to delegate it to a DPE when you take a checkride.

The flights themselves are not an issue, make sure you are current, proficient (ie not rusty) and be prepared to fly normal procedures. I had to estimate distances to objects on the ground and spot traffic, all normal for safe operation.

The process is to make an appointment with the FSDO and have an aircraft available. The medical folks will then make their decision based upon his report. It can be done in a day with planning ahead.
