After some research, I found the following key URLS:

A ground school from Gleim Aviation:

FAA AC 61-35 outlining the conversion procedure:

An important point is Section 7 concerning the requirement for submitting a Verification of Foreign License, Rating and Medical Certification form, which must be done 90 days in advance of visiting an FSDO.
