Classic Learjet (the 60 is the last real Lear) door warning indication systems are retarded. The locking mechanism is very robust. Wonder if they had been having an indication issue and decided to fly anyway? Regardless, I can't imagine a scenario with the handle properly stowed that would allow the door to open and depart.
