The page is titled: "Update the personal details / address on your Flight Crew Licence".

Form SRG2156 is titled "Application for the Notification of a Change to Personal Details, Including Change of Address, Change of Name andChange of Nationality".

Nothing suggests to me that these only apply to JAR-FCL licenses.
