I had trashed it too.
I talked about:
Lack of respect from senior management
always hearing "if you don't like it you can leave"
cost neutral proposals denied on the whim of senior managers
the HR manual being an illusion
no investing in employees to motivate them
I have never used "strongly disagree" so often in any survey
