Just to add to disc. app. proc, it is now in FCOM (Rev 6th March 2014), chapter: PRO-NOR-SOP-Approach-Approach General subchapter "Discontinued Approach".

Also includes a new callout (Airbus SOP) "CANCEL APPROACH".
