The problem is not the book, the problem is incompetent pilots!
A two engine go-around was called, the reason doesn't matter. It's a basic flying manoeuver, with or without the autopilot. But maybe the crew didn't notice the FMAs or misheard the non-excestant autopilot disconnect warning horn....
