SS, if you're in the mood to dismiss various hypothesis, how about a Kegworth? In other words, No 1 starts failing/fails, No 2 is shut down.
I know of at least one crew that did that: made a mistake on an engine malfunction, heading back to safe landing site, and before they got it all done, got the wrong engine lever moved back.
It didn't end well.
