FAA IR to UK IMC rating

I recently got my FAA instrument rating, and I also have an EASA PPL issued by the UK CAA. I'd like to add an IMC rating based on my FAA IR, in which case no skills test and written exam are necessary if the FAA IR checkride is less than 24 months old (cf. CAP 804 [1], section 5, part E, 3.3.2, "Credits"). However, but I'm unsure what specific application process applies for this conversion route, and I've been trying in vain to get information about that from the CAA.

Should I send the regular IMC application form SRG1125 [2] to the CAA together with my logbook and copy of my FAA certificate and hope for the best, or is there another procedure that I missed?

Perhaps somebody who already did this already can provide some hints? Thanks!
