I am not aware of any incident in North American airspace where a North American crew followed an ATC instruction that was contrary to TCAS RA direction. Yet it has happened at least twice in EU airspace, once fatally and here only luck avoided another tragedy.
Why is that ? Is it Aircrew training, ATC training/procedures or something else ?
