I can't speak for Pakistan, but my experience in Vietnam 5 star hotels were somewhat problematic for food poisoning, whereas the street cafe's were safe (never had an issue with street food, but then my stomach is armor plated anyway.
I put this down to the huge menus and limited custom of the big hotels requiring extended storage of food vs the morning fresh and local reputation required by the street cafes to stay in business.
Caveat, intestinal worm egg ingestion is suggested as a potential issue in SE Asia regardless where you eat, the eggs stick to stuff like lettuce.
Ascaris was quoted as the main issue. If suspected with strange poos, simply dealt with with tablets
Had a couple of suspected occurrences, but no depository evidence to suggest any infection. Wikipedia makes is sound worse than the doctors description.
