You do not have a lot of options. There is only one place at which you do your medical check and that is at the Ministry of Civil Aviation, Cairo, Egypt. From there, you can issue your Egyptian medical certificate. They also have a department that can issue FAA medical certificates if you intend on flying in the USA.
