"D15" and "D10" on the approach chart means that you use DME. At low altitude, there is not enough difference between slant range and ground distance to matter anyhow. Either use the HOLD page as usual, or enter the fixes as radial/DME if you think you can manually steer the airplane more accurately than the FMS and autopilot.

BTW, are you a pilot, or is this a flight sim question? If flight sim, please make it clear from the start.
