You need:
1)fill a request form (solicitud de nota informativa de licencia de piloto), which can be downloaded here:

2) you have to pay a fee (tasa 19ª)........ 36,64 euros

Also there´s a bank account number where you can transfer the money :
0182-2458-1002-00000489 (Banco Bilbao Vizcaya Argentaria), AESA.
3) Send the filled request form and a proof of the money transfer to:
AESA, Division de licencias al personal aeronáutico.
Avenida General Perón n.º 40, Portal B, 1ª planta. 28020 (Madrid, España).
