Accident Investigation Survey

Dear PPRuNe members

Can you please spare 15 minutes of your time to complete the following survey:

This is a university supported research project to determine what pilots’ attitudes are towards the current accident investigation process in Canada, and whether that process can be improved. This survey is open to Canadian licensed Commercial or Airline Transport Pilots, regardless of whether you fly airplanes or helicopters. Only these pilots have been asked to participate for now. A sincere apology for not including Private Pilots, Flight Attendants, Maintenance Engineers, Air Traffic Controllers and those persons strongly interested in aviation that are not pilots.

This research is completely anonymous. You will not be asked for your name, nor will you be asked for your employer’s name. The survey has been programmed so it is not possible to identify what computer was used by the pilot participating in the survey. The researcher in charge of this survey is committed to protecting your anonymity. The sponsoring educational institution imposes significant penalties on researchers who do not protect participants' anonymity.

This survey contains 27 multiple choice and single line questions. It closes at 10am Pacific time, Monday April 29th. So there is lots of time to do it whenever convenient for you. When you are finished, you will be able to see the results of 15 questions collected to date, including a breakdown for each individual question.
