Surveillance Minimum Altitudes (as on the SMA Chart) also complicate things. Either ATC discontinues the approach before the SMAs (as opposed to the MSA...), alternatively the aircrew fly the IAP, then the MAP if necessary back to terrain-safe altitudes.
