The form for everything FAA rating-related is the 8710-1.

I'm not clear if what you're asking to do is to complete a practical for a ZA ATPL and then apply for a FAA ATP type rating based upon that, or if you're trying to do a FAA Part 135 practical with a FAA DPE who just happens to be in ZA.

If it's the latter, the DPE ought to know how to do it. If it's the former, it's the 8710-1, but you might have trouble getting the FAA to buy off on it.

Trying to do much outside of a normal lane with FAA Airmen Certification is seldom easy, no matter how simple it may appear. I speak from experience.
