LPC / EASA ATPL(H)

I apologise if this has been asked before but I have searched and cannot find the answer. The new CAA EASA forms are a little confusing to this easily confused individual so perhaps someone would be so kind as to point me in the right direction.

I hold a UK JAR ATPL(H) which is valid until 2015. I have just completed an LPC revalidation and am trying to work out which forms that I downloaded from the CAA website are the ones that I need to send to CAA for the LPC revalidation. (The TRE was a non-UK examiner who had not completed the new CAA EASA forms before)

I have completed the following:

1. LPC SPH/MPH Issue 5 (SRG\1174) which has been completed by the TRE.
2. SGR 1173C Issue 01 which has also been completed by the TRE.
3. SRG 1104 Issue 08 (Application for EASA Part-FCL Licence)

I am assuming that as we are past the changeover date I must apply for licence conversion even though my JAR licence is still current?

Also previously I had always been under the impression that an LPC revalidation was free but it now seems that there is a charge of £86 - along with the £136 for the licence conversion?
