Doesnt this kind of thing almost always happen with new engine types? Thats a rhetorical question... yes it does.
I beg to differ.

For what it's worth: Here's an accident report with a remarkably similar root cause. It happens to be a skipped manufacturing step in a helical-splined piston, in an established propeller product (not a new design).
