There have been a number of unexplained in-flight fires, but none that I am aware of has ever been definitely proven to have been caused by cigarettes. The two most commonly cited examples are the
Varig 707 and
Air Canada DC-9
While the first remains unsolved, the second one had an entirely different cause, urban legend notwithstanding.
