For me personally I would not apply. If a company can not advertise who they are and be honest with a persons name and contact details, why give them your CV with your personal details.

Whats with this cloak and dagger advert
I have the same opinion. I once replied to an advert, giving my first name only and a PO box saying that when they identified themselves, so would I.
Needless to say, no response.
