If a small regional or a GA charter outfit had 10 engine failures in a couple of years CASA would be pulling the AOC.
How about a half HICAP/half charter Dash 8 outfit currently hiding beneath the radar but for not much longer?
Let's say for 'sh*ts and giigles' an airline that has had numerous investigations from CASA since January 2011, some continuing, but nothing gets done?
Let's say an airline who had a rogue HOFO at the helm and for whom it took well over a year to push aside because he is a protected species and it took some pressure from CASA to get him pushed. The same rogue is still making HOFO decisions, advising the CEO and COO, interviewing and hiring pilots without the new HOFO knowing about it? Oops, a breach of the law.
The same airline with multiple investigations taking place internally and as a result pushed and punted the safety manager so that the investigations could be covered up? Some of those investigations surround the former HOFO, the protected one. Oops. That is a little naughty.
