Don't forget I am ex RAF ground crew, I agree there should be procedures in place, but we simply do not know if that was the route cause, no one does yet, things get missed when pressure is on and people are getting rushed to do a five minute job in three. Nothing man has ever done has been 100% we are all falible from the person in the cockpit through to the engineers working on the aircraft..
