Fortunately, due to redundancy concepts, we can have small failures and not have an accident. Our actual airline accident rate on an hourly basis is far better than 6 sigma. (For most airlines at least)
Compare that with the overall Navy accident rate during most of my career--3.5 to 2.5 accidents per 10,000 flight hours. Makes one wonder if you will personally get involved in the statistics during your career.
Then add combat hazards to that.
