How about:
Overweight take off;
Not keeping good look out;
Marking the map incorrectly;
Flying through cloud;
Syphoning petrol from a jerry can on the back seat (in front of the hot exhaust)
Flying while clearly medically unfit;
and generally being a danger to anyone else who is unlucky enough to be flying anywhere near them.
