Here's another one, the 747 Halifax accident from 2004.

After the Halifax accident, takeoff performance monitoring was extensively discuss in that accident thread. However I don't recall that an adequate solution was found.

As I think about it now, an appropriately sensitive horizontal accelerometer tied into the FMC should work. All the FMC has to do is calculate an expected acceleration range for the entered weight and conditions and compare it to the measured acceleration, after the throttles move forward and the aircraft starts rolling. Modern airliners already have a vertical accelerometer, so why not add a horizontal one and add the software and crew alert logic to the FMC. This software use should be limited to the WOW condition only.

Come to think of it, the IMUs should be able to provide the acceleration data.
