Uninstall Outlook.
Remove Outlook items from the registry.
Reinstall Outlook.

What it currently sounds like is that the reinstall of Outlook is finding the original Outlook install and not overwriting whatever's at fault as a result. It's either that, or a corrupt PST as Spurlash says (in which case, try scanpst.exe against it).
