I'm not sure why people persist in labeling this event, and many others both fatal and non, as "
incidents". They meet all practical and legal definitions as "
accidents" don't they?
(Except perhaps when safety-related SOPs are disregarded, in which case I call them "accidents looking for a place to happen...")
