Ewen, I'm basing it on a detailed article (over several pages, IIRC) in Aviation Week following publication of the accident report. That's what was said at the time.
Was there later evidence, then?

Wikipedia says that the official USAF conclusion is mechanical failure, but also says that many at the time nevertheless considered it to be CFIT.
