A 'near miss' is when it was considered that there was a serious risk of collision. I have only ever declared two. One was a military radar error, heavy four jet on heavy four jet, IMC, 5 seconds separation crossing. The other was a light twin against a glider where it shouldn't have been. Now that one was close - 1m
