Hello to all...

I do the Type Rating with Flight Savety in Wichita.

For this I have 3 questions:

1. To finish the form, I need a "JAA Initial Pilot Logbook Declaration Form". Does someone know where I can find this form?

2. After the Sim Course I need 6 Take off/Landings with the plane. Can I do this with an N reg Aircraft?

3. Is that right, that I do not need a active IR? I have a MEP(land) and a ME IR but both are expired.
