National Airlines DC10 with uncontained failure over New Mexico in 1973...
Directly caused by a FD crew member disconnecting autothrust in the cruise, then re-engaging same later on...without due regard.
IE: finger trouble.

American Airlines DC10 lost engine from wing on take-off from Chicago 1979...
Directly caused by inappropriate maintenance actions.

Remember QF off the end with a 747 at BKK?
Not so bright on the part of the FD crew, now was it?
Flap 25 landing, idle reverse only....wet runway, long float, excessive speed.

A major FU of the highest order.... seems to me.
Rather a large repair bill.

So, as we can see, it's not just 'third world' operators that have problems.
