Directly caused by a FD crew member disconnecting autothrust in the cruise, then re-engaging same later on...without due regard.
IE: finger trouble.
American Airlines DC10 lost engine from wing on take-off from Chicago 1979...
Directly caused by inappropriate maintenance actions.

Flap 25 landing, idle reverse only....wet runway, long float, excessive speed.
