I holiday'ed in the states a few months back and found that the annual travel insurance I have doesn't cover flying - I bought a policy from this lot to cover the times when I was flying:

It was something like £25-30 to get two weeks cover. Price seemed good but (thankfully!) no idea if the service is any good if you have to claim.
