loading passengers and cargo in multi engined helicopters and fixed wing aircraf

I was looking for some of our friends to educate me about loading passengers and cargo in helicopters and fixed wing aircrafts when flying over sea and land. The question is "Whether to load these machines so as to maintain level flight required for diversion in case an engine quits in air?" Though this may be ideal, but this arrangement may not allow maximum exploitation of the machine you are flying. What does the regulation say in these matters? Do they have different schemes for different terrains viz seas, mountains, deserts etc. Pilots with varied experience may also add value to their comments by sharing their real time stories
