that won't happen....costs far too much.
So, expect the same sort of automation-induced accidents/incidents...perhaps even more.
The only thing that is likely to change this unfortunate picture is for the concerned regulatory authority to say....enough is enough, and lay down the law....IE: train to
proficiency.
