Not necessarily so. Awake intubation is indeed possible, and in this context not uncommon. Some lignocaine spray and a tickle of Ketamine is often required.
Ah yes! A spray of local and a "tickle of ketamine" - and if they still gag, stick'em with a bit of sux!
