I'm not privy to any details as to how this accident happened, or who is at fault.
My experience both within and without the aviation industry is that with most cock-ups the individual at the end of the chain is in the spotlight, and this focus is used to absolve those higher up the chain from any shared responsibility, or from doing anything
meaningful to prevent a reoccurrence.
