Normailzation of Deviation
I am an ex military pilot now working in another industry (Petro-chem).
I need to put together an awareness programme on Normalization of Deviation. Whilst I am aware of most of the major material out there in the aviation world (NASA, Air France etc) I am interested in personal stories where lack of adherence to procedures either did - or nearly did - result in serious consequences.
