Similarities with AA accidents and many other overruns – plan continuation bias (press-on-it is).
We need to ensure that pilots have appropriate knowledge about the risks when in landing in poor conditions and how to use that knowledge – 'know what' and 'know how' – risk management, how and when to think about these issues.
