so the first occurrence of 'pilot error' for these two was cashing their pay checks it seems.

do need to think -they sure learned their lesson huh?
So maybe the justification for putting in unambiguous alarm systems is not to coddle unthinking pilots but to save lives. Who knows, perhaps with an audible alarm,
these pilots would have put the masks on backwards...
