1. In Column 1, should the 'Month' be written in numbers or words?
usually, wat i've seen is that the month is entered in words..ie APR,JUN etc.. also a new innovative style that i've seen and is acepted by DGCA is to write the name of the month vertically in the column in full.
eg.

I
L .. so dat u dont hav to write the name of the month repeatedly..
2. In Column 2, what goes under 'Type' and 'Engines'?
Type- PA28, C172, C152.
Engines- Single/Multi
3. Are actual names expected in columns 3 and 4?
yes they are expected.
write in the pilot/instructors name. nd wherever your name comes ,put in SELF.
nd u cn just write in ur instructors initial with surname in the other column. eg. J Doe
4. Are 4-letter airports (KMIA) acceptable in Column 5 under 'From' and 'To'?
yeah, they are acceptable.. kmia, klax, kftw ..
