Well on planet earth a Pilot is one part of a whole process…the cheese “blocks” …. Before the pilot are ops, planners, SMS, CRM, Training etc… after are SMS, engineering, CRM, Training etc… meaning that in a self regulating industry it is almost impossible for a single action to cause an accident, normally a single point accident is an act of negligence or sabotage.
If a pilot hasn’t been negligent, and hasn’t been today’s saboteur then it is reasonable to suggest that the SMS, CRM, Ops manual, planning etc did, or did not do something that affected the pilots ability to carry out his duties.
Don’t get me wrong here I do realize that the accident rates are pointing at Pilots acting “outside of the box” but that is in itself a failure of the whole process not just of the pilot.
The reason model is exceptional and if you don’t get it then I suggest you get yourself off for another round of Human Factors training.
