All seperate incidents with one poor unfortunate being on the receiving end of 2. However after the repercussions of the first one it wa unlikely that he would even contemplate flying the aircraft without engineering input, no matter what the Ops Mgr thought.
The whole sordid saga has not gone unnoticed by regulator who has taken action despite this being kept under warps by said operator. I believe they have been asked to give 10 good reasons....
