To add something .... all ATC really want to know is do you have a problem up there ... and if so,... how urgent is it before you require assistance ??
Most 'medical emergencies' I seem to see overfly numerous facilities with top class medical facilities before they choose to land exactly where they originally intended to............. why ?? If the pax are that seriously in danger of losing life then surely a lengthy transit ain't going to help too much
