Just like you have certain forms/documents to be filled out and signed by your examiner in your Canadian PPL/IR/CPL checkrides, the DGCA requires you to get certain forms filled out and endorsed by your Instructor/CFI in the US or Canada etc ..
These forms are the CA39, CA40A, CA40B etc..
They are also known by other names like GFT forms, Day/Night/IR checks, Pilot Proficiency check forms etc.

If you do a google on the CA39 etc you might find links to some websites which have these forms which you can download.

You'll also find (with some research) that the format of this CA39 form varies (depending on where you downloaded or obtained it from) ..
It does not matter what format you use as long as it has all the information the DGCA needs.

The forms in that site have been tailor made for that particular flying school in the US (Monarch Air) for which they are advertising .. but you can edit it and customize it if you want.
note that those forms are in the PDF format and hence, if you're going to edit them, you'll need some kind of a PDF editor (your Adobe acrobat reader can only read those files NOT edit them).
Try Foxit PDF editor.
