"Thought for Today" on Radio4 this morning was based on "Systemic Failure" as illustrated by this and other avoidable accidents featured on this forum. The conclusion that the speaker drew was that a sign above every head honcho's door should read, "Yes means Yes, and No means No". A marked contrast to that adhered to by the MOD as you describe it, where when signing off airworthiness enforcement, "Yes" means "No". This system is totally discredited and very dangerous. It must be replaced with an independent MAA ASAP.
